Know that mediation is a voluntary process.
Review information about how mediation is a better alternative to arbitration.
Fill out the Request for Mediation form with your agent and fill in the commission you believe you are due.
Read and fill out the Agreement to Mediate form with your agent under the section labeled, 'Complainants.'
Attach all three forms to your arbitration request form and statement (labeled Exhibit 1) and any additional information that you may have.
